Our insurance coordinator, Tera, will gladly assist you in answering any questions you may have about your insurance as well as process your insurance claims.

Are you covered?

Due to the complexities of insurance contracts, please contact us directly and our insurance coordinator, Tera, can verify your coverage levels.

New patient?

Please bring your current insurance card with you for your first visit. If your insurance requires a referral, then bring the completed Insurance Referral form to your appointment as well.

New insurance?

Please contact us if you are a returning patient who has changed insurance carriers since we last saw you. We’ll gladly update your files to make sure you are receiving your new carrier’s benefits.
